Philippine NBI Clearance Certificate Application

NBI customs clearance (important note: the applicant can be a foreigner living in PH, and his/her fingerprint card can be obtained, filled in and authenticated by the embassy, but the actual NBI customs clearance certificate must be obtained by the authorized representative of the applicant of the electronic data processing department [EDPD] located in NBI Taft Avenue, Manila.)

For first-time applicants for NBI clearance certificates:

1. Get the fingerprint card form from the embassy and fill in the required personal data on both sides of the card truthfully and clearly.

2. If you are a married woman, your name is written in the following order:

A. Your maiden name (father's surname);

B. Your husband's surname;

C. Your name or Christian name

d. Your mother's surname (your mother's surname)

3. Attach the latest copy of your 2 "x 2" white background photo (taken within 3 (3) months before application).

4. Print your fingerprint in place (ROLLED IMPRESSION).

5. Make the consulate authenticate the fingerprint card form so that the document is considered to be legally used in the Philippines. Once certified by the embassy, the application is considered "completed".

6. Sending instructions: The completed form can be sent to NBI through the representative of the Philippines. Send the completed form to your representative together with a signed letter authorizing him/her to handle business on your behalf. This power of attorney must be consul/legalized by the Philippine embassy. Your relatives can go to the Electronic Data Processing Unit (EDPD) office at NBI Taft Avenue in Manila for further assistance.

7. After receiving your NBI customs clearance certificate, please check whether there is embossed NBI DRY SEAL at the bottom left. Without it, the license is invalid. Before submitting it to the end user, you must also paste your clear and correct fingerprint in the blank space provided on the customs clearance certificate.

For applicants who renew the NBI clearance certificate:

If you have the original personal copy of the NBI license issued from 2014 onwards, simply send the certificate to the NBI office for update, together:

1.2 "x 2" white background photo (taken within 3 (3) months before application);

2. PHP 200.00 costs.

3. A copy of the passport, stamped with a date to prove that it is abroad;

4. Update any information that needs to be updated on your old customs clearance certificate. If there is no change, write the same data. The update only allows change of address and customs clearance purposes; 5. If your name, date of birth and place of birth

If there is a change, it is recommended that you fill in the new applicant form and attach a copy of your birth certificate or marriage certificate as proof of the change. 6. Follow the same SENDING instructions as above.

If your permission was issued before 2014, or if you have lost a personal copy of the certificate, please follow the instructions of the first applicant.
