2. Two (2) completed visa application forms with photos (double-sided);

3. Confirmed voyage/ticket and its copy;

4. The employment certificate/letter of introduction of the employer company translated in English or English, accompanied by the signature of the authorized person of the company and the company seal/seal, the invitation letter of the Philippine Shipping/Maritime Company; and

5. Visa fee: RMB 144.00
