What materials do international travelers entering the Philippines need to submit to apply for One Health Pass?

"One Health Pass" is abbreviated as "OSS" health code. All international passengers entering the Philippines must apply for OSS health code in advance, which is equivalent to the passenger's health statement. International passengers submit materials to apply for OSS health code, and then they will get a two-dimensional code, as shown in the following figure:
