1. Provide at least one valid ID with photos. You need to identify yourself with an ID. In the whole application process, it is needed to fill in the form and other links. The ID must be issued by the government with your photo on it. Valid identity documents issued by the Government include:

Digital Social Insurance Identification Card (SSS)
Government Service Insurance System Electronic Card (GSIS)
Professional Management Committee Identification Card (PRC)
Philippine United Bar Association Identification Card (IBP)
Overseas Labor Welfare Administration Identification Card (OWWA)
Digital Internal Revenue Service Identification Card (BIR)
Senior Citizen Card
The old university student card, alumni card, and old employment identification card are also valid identity documents.
2. In addition, you need to prepare three supporting documents to prove your citizenship together with your photo ID. 3. Carry the passport and copy of the latest invalid or about to expire. The data information page on the passport should also be copied and carried.
If your passport is brown, make copies of pages 1 to 3.
If your passport is a green passport, copy the first page.
If your passport is a machine-readable passport (MRP), copy the first and last pages.
If your passport is an e-passport, copy the first and last pages.
4. Ask the National Bureau of Statistics for proof of your change of surname. If you are married, the Ministry of Foreign Affairs requires several certificates to change your surname. It is available on the website of the National Bureau of Statistics.
Married women or persons married to foreigners who want to use their marital surnames need to submit a marriage certificate issued by the National Bureau of Statistics or a certificate issued by the Local Civil Registry (LCR) and certified by the National Bureau of Statistics.
Expatriate spouses will also present the original and photocopy of the Philippine Overseas Commission (CFO) Guidance Consultation Course Attendance Certificate with their marriage certificate.
Persons married overseas are also required to submit a marriage report certified by the National Bureau of Statistics.
5. Restore the maiden name. To change the surname to the birth surname, a birth certificate and other documents issued by the National Bureau of Statistics are obtained based on marital status.
If a married woman wishes to restore her premarital name, she should provide a birth certificate issued by the National Bureau of Statistics.
A bereaved woman should provide a death certificate of her deceased husband issued by the National Bureau of Statistics.
Divorced persons are required to provide a certified copy of the divorce book. This copy may be authenticated by the Philippine embassy or consular mission in the Philippines where the divorce is located. It is also necessary to attach the marriage certificate of the National Bureau of Statistics after the divorce decree.
If the marriage is found to be invalid, a certificate of marriage with the National Bureau of Statistics and a court order certification to invalidate the marriage are required.
6. Change your name to your Muslim name. If you are a convert and want to use a Muslim name, you need to obtain a raised birth certificate from the National Bureau of Statistics, which will show your Muslim name. You will also show the conversion certificate of the Philippine Muslim National Council (NCMF) or the Muslim Affairs Office (OMA).
If you are a Muslim but do not need to change your name, you can prepare your passport according to the general requirements of valid identity documents and auxiliary documents, including recently expired or expired passports.
7. Minors need birth certificates and parents' supporting documents. The renewal of a passport by a minor is regarded as the first application and a birth certificate from the National Bureau of Statistics is required. Parents or guardians need to be present at the same time during the appointment processing time, and at the same time bring the parent's marriage certificate, the original and photocopy of the passport of the person traveling with the minor, and the minor's previous passport.
8. Copy all identity documents and original documents. Make at least two copies of each document. The Ministry of Foreign Affairs will not accept the original and copy when traveling to the Ministry of Foreign Affairs.
